    • The number in which the judge issued an order denying an abortion without notification; and
    • The number of denials from which an appeal was filed.
  • Requires the Department of Public Health to prepare a reporting form for physicians who perform abortions (Sec. 1).
  • Specifies that individual reports shall be confidential and not subject to disclosure (Sec. 1).
  • Requires individual reports to be destroyed six months after submission to the Administrative Office of the Courts (Sec. 1).
