When I set a goal of 100% accuracy in my typesetting business, I got everybody involved to sit down together. Instead of blaming each other, the proofreaders and the typesetters came up with a new procedure. They bought a 29 cent highlighter. We reached our goal.
Fixing education is going to involve a lot more than a yellow marker, but the collaborative approach is proven to work.
