- -A sworn statement regarding the candidate's citizenship, age, and places of residence in the United States for 14 years; and
-A certified copy of the candidate's long form birth certificate or, if the long form birth certificate is not available, he or she may submit 2 or more of the following documents:
- -An early baptismal or circumcision certificate;
-A hospital birth record;
-A postpartum medical record for the mother or child; or
-An early Census record.
