Title: Requiring Women Seeking Abortions to Answer Questionnaire
Vote to pass a bill requiring physicians to complete a specific form when performing an abortion or when discovering a medical complication caused by a past abortion and requiring the State Department of Health to produce reports with the data thus obtained.
- -The date and the county where the abortion was performed;
-Information regarding the woman having aborted: age, marital status, race, level of education, state or foreign country of residence, previous pregnancies;
-The approximate gestational age of the embryo/fetus;
-The method that was used to perform the abortion;
-Whether the abortion failed and led to the birth of an infant, and if yes, whether measures to keep him alive were undertaken and how long the child survived;
-The reasons given by the woman for having had an abortion;
-Whether ultrasound equipment was used before,during, and after the procedure;
-Whether the woman seeking an abortion has been provided the information required by law;
-In case the woman having aborted was a minor: her age, the age of the father of the fetus, whether one of her parents was notified, whether the informed written consent of one of them was obtained, and if no, by which means was the minor legally able to get an abortion;
-The date at which the form was completed; and
-The name and license number of the physician.
- -The name, address and telephone number of the health facility where the complication was discovered;
-The date on which the complication was discovered;
-The health facility where the abortion was performed as well as the date of the procedure;
-The age of the patient;
-The nature of the complication; and
-The name and license number of the physician.
NOTE: A TWO-THIRDS MAJORITY OF MEMBERS ELECTED IS REQUIRED TO OVERRIDE A GOVERNOR'S VETO.
