Highlights:

-Requires a physician who is performing an partial birth/late term abortion to report the following (Secs. 1 and 4):

-The medical basis and reasons, including the specific medical diagnosis, for the determination that an abortion is necessary to preserve the life of the pregnant woman or that the continuation of the pregnancy will cause "susbtantial and irreversible" impairment of a major bodily function of the pregnant woman; -The name of the referring physician; and -A sworn statement by the physician performing the abortion and the referring physician that such physicians are not legally or financially affiliated.
